Sam's brother is the family doctor for an aging and whalthy man Ian. After the doctore passed away, it is Sam's nephew - a new medical graduate - to take care of Ian's health. One day the nephew had a drink with the uncle at a bar and boasted how he had cured Ian of his chronic violent cough.
